Job summary

Northshore Home Health Care is seeking a Licensed Physical Therapist to provide in-home and senior living care under the Geriatric House Calls model. You will perform evaluations, craft personalized treatment plans, and deliver evidence-based therapies to improve mobility, independence, and quality of life.

Responsibilities include documenting care in EMR, collaborating with PTs, OTs, SLPs, and fitness staff, and educating patients and caregivers to promote aging in place.

Qualifications

• Must hold valid Physical Therapy license or eligibility in the practice state.
• Degree from an accredited Physical Therapy program.
• Proficiency in computer use and EMR documentation.
• Current CPR certification.

Licensed Physical Therapist providing personalized, in-home and senior living care to older adults, utilizing the Geriatric House Calls™ model.

Physical Demands

Ability to lift up to 50 lbs, assist with patient transfers, perform repositioning, stand and walk for extended periods, and drive to patient locations. Use of PPE when necessary.
