Responsibilities

• Provide one-on-one therapy sessions in patients' homes and senior communities.
• Perform initial evaluations and develop individualized care plans.
• Deliver therapy to improve mobility, independence, and overall wellness.
• Document care accurately in electronic medical records.
• Collaborate with interdisciplinary teams (PTs, OTs, SLPs, fitness professionals).
• Educate patients, caregivers, and staff on safety, function, and aging in place.
• Utilize telehealth and virtual care to enhance access and flexibility.
