Qualifications

• Graduate of an accredited PT program with state license.
• Current PT license for Virginia.
• Ability to conduct PT assessments and provide direct patient care.
• Familiarity with PCC EMR (training available).
• CPR certification required.

Responsibilities

• Provide PT assessments of referred patients and plan rehab.
• Deliver direct patient care and monitor progress.
• Document in PCC EMR.
• Collaborate with team to optimize patient outcomes.

Qualifications:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Valid Physical Therapy (PT) license for state practicing.
• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation (we use PCC but experience in this system is not required).
• CPR Certification.
