Qualifications

• Graduate from an accredited Physical Therapy program.
• Valid PT license for state practice.
• Conduct PT assessments of referred patients delivering direct patient care.
• Proficient in EMR documentation (PCC) preferred but not required.

Responsibilities

• Perform patient evaluations and develop rehabilitation plans.
• Provide direct patient care and monitor progress.
• Document care in the EMR and coordinate with care teams.

Qualifications:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Valid Physical Therapy (PT) license for state practicing.
• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation (we use PCC but experience in this system is not required).
• CPR Certification.
