Qualifications

• Licensed Physical Therapist with a degree from an accredited PT program.
• Current PT license for the state of practice.
• CPR certification is required.
• Proven ability to perform PT assessments and patient care.

Responsibilities

• Conduct PT assessments of referred patients and provide direct patient care to assess medical conditions, functional capabilities, limitations, and rehabilitation potential.
• Document patient progress thoroughly in PCC EMR.
• Collaborate with a multidisciplinary team to optimize outcomes.

Qualifications:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Valid Physical Therapy (PT) license for state practicing.
• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation (we use PCC but experience in this system is not required).
• CPR Certification.
