Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist (PT) to join our in-home health team. You will assess clients in their homes, develop personalized care plans, and deliver skilled PT interventions to improve strength, balance, mobility, and safety.

This part-time/per diem role offers competitive pay ($75–$85/hour) with mileage reimbursement, flexible scheduling, and the support of an experienced office and clinical leadership.

Qualifications

• Active MA PT license
• DPT or MPT from an accredited program
• CPR certified
• 1+ year of experience preferred (new grads welcome)
• Reliable vehicle & valid driver’s license

Responsibilities

• Perform in-home PT evaluations and create personalized care plans
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety
• Educate clients and families on exercises and home safety
• Collaborate with a multidisciplinary care team
• Document care using mobile-friendly EMR
