Job summary

Rock Health is seeking a Licensed Physical Therapist to provide in-home and senior living care to older adults, applying a flexible, patient-centered approach. You will evaluate patients and design personalized treatment plans to improve mobility, independence, and quality of life.

Responsibilities include delivering therapy sessions, documenting care in EMR, collaborating with interdisciplinary teams, and educating patients and caregivers. Telehealth options may be incorporated when appropriate.

Qualifications

• Valid state license or eligibility to practice physical therapy.
• Degree from an accredited physical therapy program.
• Proficiency with electronic medical records (EMR) and basic computer skills.
• Current CPR certification.

Job description

Role:

Licensed Physical Therapist providing in-home and senior living care to older adults, utilizing a flexible, patient-centered approach.

Responsibilities:

Conduct evaluations, develop personalized treatment plans, deliver evidence-based therapy sessions to improve mobility, independence, and wellness. Document care accurately in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.

Qualifications:

Valid state license or eligibility, degree from an accredited program, basic computer and EMR proficiency, current CPR certification.

Physical Demands:

Ability to perform moderate physical effort, assist with patient transfers, reposition patients, stand for extended periods, climb stairs, and drive between locations. PPE use required as needed.
