Document patient progress accurately, collaborate with interdisciplinary teams, and educate patients, caregivers, and staff on safety and aging-in-place strategies. Support virtual care when appropriate.

Qualifications & Skills

Valid PT license, degree from an accredited program, proficiency with EMR and computers, and CPR certification. Ability to meet physical demands including lifting, repositioning patients, and driving to care locations.
