Job summary

LiveWell Homecare Agency, LLC seeks licensed Physical Therapists to provide individualized, one-on-one in-home care to older adults within New Jersey. You will conduct evaluations, design personalized plans, and deliver evidence-based therapy sessions in private homes and senior living communities.

You’ll document care in EMR, collaborate with an interdisciplinary team, and educate patients and caregivers on safety and aging-in-place strategies.

Qualifications

• Licensed Physical Therapist in the practice state or eligible
• Degree from an accredited Physical Therapy program (DPT)
• Proficiency with computers and EMR systems
• Current CPR certification

Responsibilities

Job description

We are seeking licensed Physical Therapists to provide individualized, one-on-one care to older adults in private homes and senior living communities, utilizing our Geriatric House Calls™ model.

Responsibilities include:

• Conducting evaluations and developing personalized treatment plans
• Delivering evidence-based therapy sessions to enhance mobility, independence, and quality of life
• Documenting care accurately within electronic medical records
• Collaborating with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists
• Educating patients, caregivers, and staff on safety and aging-in-place strategies
• Providing telehealth and virtual care to increase access and flexibility

Qualifications and skills:

• Valid Physical Therapy license or eligibility in practice state(s)
• Degree from an accredited Physical Therapy program
• Proficiency with computers and EMR systems
• Current CPR certification

Physical requirements:

• Ability to exert moderate physical effort, including lifting, repositioning patients, and assisting with transfers
• Capability to perform CPR and navigate patient environments
• Willingness to drive to patient locations and wear PPE as needed
