Job summary

LiveWell Homecare Agency, LLC is seeking a licensed Physical Therapist to provide personalized in-home and senior living care to older adults, with a flexible schedule and telehealth when appropriate. Responsibilities include evaluating clients, developing individualized treatment plans, and delivering evidence-based therapy.

You will document care in the EMR, collaborate with interdisciplinary teams, and educate patients, caregivers, and staff to promote safety and aging in place.

Qualifications

• Valid PT license
• Degree from accredited program
• Basic EMR proficiency
• Current CPR certification
• Ability to perform in-home care tasks including patient transfers and driving between locations
