Job summary

Healthcare Deciphered seeks a licensed and experienced Physical Therapist to provide outpatient rehabilitation services in an in-home setting for older adults, across private residences or independent living facilities.

You will assess and deliver PT programs, educate patients and caregivers, and coordinate referrals as needed, while promoting independence and overall well-being within a flexible schedule.

Qualifications

• Valid Physical Therapy license in the applicable state(s) or proof of eligibility.
• Master's degree in Physical Therapy (or a related field) or a Bachelor's degree in Physical Therapy. Degrees earned outside the U.S. must be evaluated for equivalency.
• Minimum of 5 years of progressive experience in Physical Therapy.
• Valid driver's license or reliable transportation for in-home patient visits.
• Ability to meet physical demands, including lifting 50 lbs and standing or walking for extended periods.
• Compliance with legal standards and policies, including HIPAA and credentialing requirements.
