Job summary

LiveWell Homecare Agency, LLC in the Augusta area is seeking a dedicated Physical Therapist to provide one-on-one geriatric care in patients' homes, focusing on functional outcomes and autonomy.

You will collaborate with an interdisciplinary team, develop personalized treatment plans, and support recovery with flexible schedules including full-time, part-time, or PRN options and sponsorship for qualified H1B candidates.

Qualifications

• Active physical therapy license or eligible for licensure.
• Degree from an accredited physical therapy program.
• Proficient computer skills and current CPR certification.

Responsibilities

• Provide one-on-one geriatric PT services in patients' homes.
• Collaborate with interdisciplinary team to support recovery.
• Develop personalized treatment plans to improve functional outcomes.
