Job summary

LiveWell Homecare Agency, LLC seeks a Licensed Physical Therapist specializing in geriatric care to provide one-on-one therapy in private homes and senior living communities.

You will conduct evaluations, develop personalized treatment plans, and deliver evidence-based therapy to improve mobility and independence, while documenting in EMR and educating patients and caregivers. Telehealth may be used when appropriate.

Qualifications

• Valid state license or eligibility to practice physical therapy in Florida.
• Degree from an accredited physical therapy program.
• Proficiency with EMR systems and basic computer skills.
• Current CPR certification.

Responsibilities

• Conduct evaluations and develop personalized treatment plans.
• Deliver evidence-based therapy to improve mobility and independence.
• Document progress in EMR and educate patients and caregivers.
• Collaborate with interdisciplinary teams and incorporate telehealth when appropriate.
