Qualifications

Responsibilities

• Perform physical therapy assessments and provide direct patient care to evaluate mobility, function and rehabilitation potential.
• Document findings and progress in the EMR.
• Collaborate with the healthcare team to optimize patient outcomes.
• Educate patients and families on treatment plans and home programs.
• Maintain accurate and timely documentation in PCC EMR.

Qualifications

• Graduate of an accredited Physical Therapy program.

• Current and valid Physical Therapist (PT) license in the state of practice.

• Ability to conduct comprehensive Physical Therapy assessments of referred patients and provide direct patient care to evaluate medical conditions, functional capabilities, limitations, and rehabilitation potential.

• Proficiency in EMR documentation. We use PCC, but prior experience with this system is not required.

• Current CPR certification.

• Strong communication, clinical, and documentation skills.

• Commitment to providing compassionate, patient-centered care.
