Job summary

Azza HealthCare Agency in New York is seeking a Licensed Physical Therapist to provide one-on-one in-home and senior living care under the Geriatric model. You will conduct evaluations, develop care plans, and deliver evidence-based treatments to support mobility and independence.

You will document care in EMR systems, collaborate with an interdisciplinary team, educate patients and caregivers, and incorporate telehealth for access and continuity of care.

Qualifications

  • Valid PT license and degree from an accredited program.
  • Proficiency in computer use and EMR systems, current CPR certification.
  • Ability to meet physical demands such as lifting, repositioning patients, and performing therapeutic activities.

Job description

Role:

Licensed Physical Therapist providing one-on-one care to older adults in private homes and senior living communities, utilizing the Geriatric House Calls™ model.
