Job summary

CARIN Alliance is seeking a Licensed Physical Therapist to provide personalized in-home and senior living care using the Geriatric House Calls™ model. You will conduct evaluations, develop individualized care plans, and apply evidence-based treatments to improve mobility and independence.

Responsibilities include documenting progress in EMR, collaborating with PT/OT/SLP teams, educating patients and caregivers, and incorporating telehealth to extend access and flexibility for older adults across

Qualifications

• Active Physical Therapy license or eligibility in practice state.
• Degree from an accredited Physical Therapy program.
• Proficiency in basic computer skills and EMR systems.
• Current CPR certification.
