Job summary

Live Well Homecare is seeking a Licensed Physical Therapist to provide one-on-one, in-home care for older adults in their homes or senior living communities.

You will evaluate, plan, and deliver therapy to improve mobility and independence, document progress, and collaborate with a multidisciplinary team. Telehealth may be used to extend reach and flexibility.

Qualifications

• Valid PT license in good standing.
• Degree from an accredited physical therapy program (DPT or equivalent).
• Proficiency with EMR systems and basic computer skills.
• Current CPR certification.
• Ability to perform physical tasks including lifting and driving to patient locations.
• Physical endurance for community-based care.

Responsibilities

Responsibilities

Conduct evaluations, develop individualized treatment plans, deliver evidence-based therapy to improve mobility, independence, and wellness. Document patient progress in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.
