Elara Caring in Mishawaka, IN is seeking a dedicated Physical Therapist to join our home-based care team. You will evaluate patients, develop individualized treatment plans, and deliver PT services in patients' homes.

The role requires an APTA-approved degree, an active PT license, and at least one year of healthcare experience. You will coordinate with physicians and caregivers, document care, and supervise therapy staff as needed.

Job Description

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support staff, as appropriate.

What is Required?

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
• Current, unrestricted PT license valid for the state of work
• Minimum one (1) year of experience in a healthcare facility
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
