Job summary

Elara Caring in Tulsa, OK is seeking a dedicated Physical Therapist to support patients in the home setting. You will evaluate patients, develop individualized treatment plans, and provide high-quality physical therapy services at patients' homes.

Collaborate with physicians and caregivers, monitor progress, complete accurate clinical documentation, and supervise therapy assistants as appropriate while upholding safety and quality standards.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
