Job summary

Elara Caring is seeking a licensed Physical Therapist to provide in-home therapy across assigned patient homes in the Topeka area. You will evaluate patients, design individualized plans, and deliver high-quality care in a home setting.

Applicants must have an APTA-approved PT program, current licensure, and at least one year of healthcare facility experience, with reliable transportation for community visits.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess a valid driver's license or reliable transportation to travel between patient homes and locations.
• Ability to frequently lift up to 50 pounds and move equipment.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support staff, as appropriate.

What is Required?

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
