Health Wealth Safe is seeking a licensed Physical Therapist to provide one-on-one care in patients' homes and senior living communities under our Geriatric House Calls™ model. The role spans evaluations, treatment planning, therapy delivery, and documentation in EMR, with telehealth options for continuity of care.

The position supports a clinician-driven environment focused on meaningful patient outcomes, health, and community well-being.

Qualifications

• Valid PT license or eligibility.
• Degree from an accredited PT program.
• CPR certification.
• Ability to lift, assist with transfers, and drive to patient locations.

Responsibilities

• Conduct patient evaluations and develop individualized treatment plans.
• Deliver evidence-based therapy and document in EMR.
• Collaborate with interdisciplinary teams to optimize care.
