Job summary

All Care and Comcare Home Health is seeking a compassionate Physical Therapist to provide in-home therapy across our service area, with a flexible schedule and supportive team. You will perform SOC, ROC, recertifications, and DC as part of comprehensive home health care.

The role requires an active California PT license, good documentation, and the ability to travel to patient homes while collaborating with an interdisciplinary team to meet individualized goals.

Qualifications

• Active California Physical Therapy license required.
• Current CPR certification required.

Responsibilities

• Provide physical therapy services per physician orders.
• Conduct SOC/ROC/Recert/DC in home health settings.
• Assess patient needs and adjust interventions to goals.

Position:

Field Physical Therapist

Key Responsibilities:

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.

Qualifications:

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout our service area.
