Job summary

Lakeport Post Acute in Lakeport, CA is seeking a dedicated Physical Therapist to join our growing rehabilitation team. We offer Full-Time, Travel, and PRN opportunities with competitive pay and comprehensive benefits.

You will evaluate patients, develop individualized treatment plans, supervise PTAs, and collaborate with a multidisciplinary team to improve mobility and independence for residents. Relocation assistance and a $5,000 sign-on bonus are available to eligible candidates.

Qualifications

• Licensed California PT in good standing.
• Bachelor's degree in Physical Therapy; master's/doctorate preferred.
• Experience in SNF/Post-Acute care or rehab settings preferred.

Responsibilities

• Evaluate patients and complete PT assessments.
• Develop individualized treatment plans based on referrals and goals.
• Provide evidence-based PT interventions to improve mobility and function.
• Supervise PTAs and rehabilitation staff.
• Collaborate with interdisciplinary teams and participate in discharge planning.
• Educate patients and caregivers; maintain accurate documentation.
• Ensure compliance with regulatory guidelines.
