Lakeport Post Acute in Lakeport, CA is seeking a compassionate Occupational Therapist (OT) to join our rehabilitation team. We offer full-time and part-time schedules with flexible options, supporting independence and quality of life for residents.

You will evaluate residents, plan individualized treatments, and collaborate with an interdisciplinary team to maximize outcomes. Relocation assistance and a sign-on bonus may be available; DOE, benefits, and ongoing professional development provided.

Qualifications

• Current California Occupational Therapist (OT) License required.
• Graduate of an accredited Occupational Therapy program.
• Strong clinical assessment, treatment planning, and documentation skills.
• Excellent communication and interpersonal abilities.
• Compassionate, patient-centered approach to care.
• Ability to work effectively within an interdisciplinary healthcare team.
• Experience in SNF, Post-Acute, Rehabilitation, Long-Term Care, or Acute Care settings preferred.
• New graduates are encouraged to apply.

Responsibilities

• Evaluate residents and develop individualized treatment plans based on functional goals and clinical needs.
• Provide occupational therapy services to improve independence with Activities of Daily Living (ADLs).
• Implement evidence-based therapeutic interventions and adaptive strategies.
• Assess cognitive, physical, and functional abilities to maximize patient outcomes.
• Collaborate with physicians, nurses, therapists, and interdisciplinary healthcare team members.
• Educate residents, families, and caregivers regarding treatment plans, progress, and home exercise programs.
• Participate in care planning meetings and interdisciplinary conferences.
• Recommend adaptive equipment, durable medical equipment, and environmental modifications as appropriate.
• Maintain accurate and timely documentation in compliance with facility, state, and federal regulations.
• Monitor patient progress and modify treatment plans to achieve optimal outcomes.
