Job summary

Lakeport Post Acute in Lakeport, CA is seeking a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team. You will help residents regain mobility and independence, working under a licensed PT to implement treatment plans and monitor progress in a collaborative, skilled nursing setting.

We offer competitive pay ($50.00-$55.00/hour DOE), sign-on bonus, relocation assistance, and both full-time and part-time roles with flexible scheduling.

Qualifications

  • Current California PTA license required.
  • Graduate of an accredited PTA program.
  • Strong communication, organizational, and interpersonal skills.
  • Compassionate, patient-centered approach to care.
  • Ability to work effectively within an interdisciplinary healthcare team.
  • Experience in SNF, post-acute, rehabilitation, LTC, or acute settings preferred.
  • Knowledge of rehab principles and mobility training preferred.

Responsibilities

  • Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
  • Implement individualized treatment plans to improve strength, mobility, balance, endurance, and independence.
  • Assist patients with therapeutic exercises, transfer training, gait training, and mobility activities.
  • Monitor resident performance and progress toward goals.
  • Communicate patient progress to supervising Physical Therapist.
  • Educate residents and families on exercise programs and safety strategies.
  • Collaborate with nursing staff, physicians, therapists, and interdisciplinary teams.
  • Maintain accurate documentation per regulatory and facility requirements.
  • Participate in care planning and discharge planning.

Physical Therapist Assistant (PTA) Responsibilities
  • Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist
  • Implement individualized treatment plans designed to improve strength, mobility, balance, endurance, and functional independence
  • Assist patients with therapeutic exercises, transfer training, gait training, and mobility activities
  • Monitor resident performance, response to treatment, and progress toward goals
  • Communicate patient progress and treatment outcomes to supervising Physical Therapist
  • Educate residents and families on exercise programs, mobility techniques, and safety strategies
  • Collaborate with nursing staff, physicians, therapists, and interdisciplinary care teams
  • Maintain accurate and timely documentation in accordance with regulatory and facility requirements
  • Participate in care planning meetings and discharge planning activities
  • Ensure a safe and positive therapy experience for all residents
Qualifications
  • Current California Physical Therapist Assistant (PTA) License required
  • Graduate of an accredited Physical Therapist Assistant program
  • Strong communication, organizational, and interpersonal skills
  • Compassionate, patient-centered approach to care
  • Ability to work effectively within an interdisciplinary healthcare team
  • Experience in Skilled Nursing Facility (SNF), Post-Acute Care, Rehabilitation, Long-Term Care, or Acute Care settings preferred
  • Knowledge of rehabilitation principles, therapeutic exercise, and mobility training preferred
