Job summary

Santa Rosa Post Acute is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team in Santa Rosa, CA. You will evaluate residents, develop individualized plans, and provide evidence-based PT services to improve mobility and functional independence.

Qualifications

• Current California Physical Therapist (PT) license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal abilities.
• Ability to work effectively within an interdisciplinary healthcare team.
• SNF, post-acute care, rehabilitation or long-term care experience preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide evidence-based physical therapy to improve mobility and independence.
• Assess needs and establish measurable treatment goals.
• Monitor progress and modify plans as appropriate.
• Collaborate with physicians, nursing staff, and families.
• Educate patients and caregivers on plans, exercises and safety.
• Supervise Physical Therapist Assistants as appropriate.
• Maintain accurate documentation per guidelines.
• Participate in interdisciplinary care planning meetings.
