Qualifications

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services in a SNF/post-acute setting.
• Improve mobility, strength, balance, endurance and independence.
• Monitor progress and modify treatment plans as appropriate.
• Collaborate with physicians, nursing, and therapy colleagues.
• Educate patients and families on goals and home programs.
• Complete accurate clinical documentation in compliance with regulations.
