Job summary

Lakeport Post Acute is seeking a compassionate Occupational Therapist (OT) to join our rehabilitation team in Lakeport, CA. This role offers both full-time and part-time opportunities with pay starting at $70 per hour, DOE, and a supportive patient-centered culture.

You will evaluate residents, develop individualized treatment plans to improve ADLs, collaborate with physicians and nurses, and educate residents and families.

Qualifications

• Current California Occupational Therapist (OT) license required.
• Strong clinical assessment and treatment skills.
• Excellent communication and interpersonal abilities.
• SNF, Post-Acute Care, Rehabilitation, or Long-Term Care experience preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide occupational therapy services to improve independence with ADLs.
• Implement evidence-based therapeutic interventions and adaptive strategies.
• Assess cognitive, physical, and functional abilities to maximize patient outcomes.
• Collaborate with physicians, nurses, therapists, and interdisciplinary team members.
• Educate residents, families, and caregivers on treatment plans, progress, and home exercise programs.
• Participate in care planning meetings and interdisciplinary team conferences.
• Recommend adaptive equipment and environmental modifications when appropriate.
• Maintain accurate and timely documentation in compliance with facility, state, and federal regulations.
• Monitor patient progress and modify treatment plans as needed.

• Provide occupational therapy services to improve independence with activities of daily living (ADLs)
• Implement evidence-based therapeutic interventions and adaptive strategies
• Assess cognitive, physical, and functional abilities to maximize patient outcomes
• Collaborate with physicians, nurses, therapists, and interdisciplinary team members
• Educate residents, families, and caregivers on treatment plans, progress, and home exercise programs
