Job summary

Lakeport Post Acute is seeking a compassionate Occupational Therapist to evaluate residents, develop individualized ADL-focused plans, and provide therapy to maximize independence. You’ll work with a collaborative rehab team to implement evidence-based interventions and educate families.

Qualifications

• Current California Occupational Therapist (OT) license
• Graduate of an accredited Occupational Therapy program
• Strong clinical assessment, treatment planning, and documentation skills

Responsibilities

• Evaluate residents and develop individualized treatment plans based on functional goals and clinical needs
• Provide occupational therapy services to improve independence with Activities of Daily Living (ADLs)
• Implement evidence-based therapeutic interventions and adaptive strategies
• Assess cognitive, physical, and functional abilities to maximize patient outcomes
• Collaborate with physicians, nurses, therapists, and interdisciplinary healthcare team members
• Educate residents, families, and caregivers regarding treatment plans, progress, and home exercise programs
• Participate in care planning meetings and interdisciplinary conferences
• Recommend adaptive equipment, durable medical equipment, and environmental modifications as appropriate
• Maintain accurate and timely documentation in compliance with facility, state, and federal regulations
• Monitor patient progress and modify treatment plans to achieve optimal outcomes
