Job summary

All Care and Comcare Home Health is seeking a compassionate Physical Therapist to join our growing team. You will provide high-quality care in patients’ homes with a flexible schedule and supportive team environment.

As a Field Physical Therapist, you will perform SOC, ROC, Recert, and DC evaluations, plan interventions, educate families, and collaborate with the interdisciplinary team to meet individualized goals across our service area.

Qualifications

• Active license/registration with the Physical Therapy Board of California.
• Excellent communication and interpersonal abilities.
• Strong documentation and organizational skills.
• Willingness to travel to patient homes for evaluations and treatment.

Responsibilities

• Provide comprehensive, skilled physical therapy services per physician orders.
• Perform SOC evaluations, ROC, Recert and DC processes.
• Assess patient needs, plan interventions, and evaluate outcomes to meet goals.
• Collaborate with the interdisciplinary team to set and review patient goals.
• Educate patients, families, and caregivers on home exercise programs.

Position:

Field Physical Therapist

Key Responsibilities:

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.

Qualifications:

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
