Remedy Home Health Care is actively seeking a licensed Physical Therapist (PT) to join our Home Health team serving Los Angeles County, Huntington Park, Maywood, South Gate and surrounding areas.

The role offers Per Diem/PRN, Part-Time or Full-Time opportunities with flexible schedules, competitive per-visit pay, and autonomy to provide skilled rehab and community-based care in patients' homes and living communities.

Qualifications

Licensed Physical Therapist (PT) in California (Required).
CPR Certification (AHA).
Minimum 1 year of clinical experience; Home Health preferred.
Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

Conduct in-home PT evaluations and reassessments.
Develop and implement personalized rehab care plans.
Improve strength, balance, mobility, and functional outcomes.
Provide therapy in home health, assisted living, and community care settings.
Educate patients and families on fall prevention and safe mobility techniques.
Document visits using WellSky (Kinnser) EMR.
Collaborate with interdisciplinary home health teams.
