Job summary

Remedy Home Health Care is seeking a licensed Physical Therapist (PT) to join our Ventura County home health team. This role offers flexible per diem, PRN, part-time or full-time opportunities to provide skilled rehab in patients' homes and community settings.

You will work with geriatric and senior populations, focusing on fall prevention, mobility, and independence, with collaboration from nurses, case managers, and physicians for comprehensive care.

Qualifications

• Licensed Physical Therapist in California (PT)
• CPR Certification (AHA)
• Minimum 1 year of clinical experience (Home Health preferred)
• Experience with geriatrics or community-based care preferred

Responsibilities

• Conduct in-home PT evaluations and reassessments
• Develop and implement personalized rehabilitation care plans

Your Role: Home Health Physical Therapist (PT)

As a Physical Therapist in Home Health , you will provide skilled rehab services tailored to each patient's needs. Your focus will include fall prevention, strength building, mobility training, and functional independence for senior and geriatric patients .

Key Responsibilities:

• Experience in senior care, geriatrics, rehab, or community-based care preferred
• Strong clinical, communication, and patient education skills
• Valid driver's license, reliable transportation, and auto insurance
