Job summary

Remedy Home Health Care seeks a licensed Physical Therapist (PT) to join our home health team in East LA County. This per diem role focuses on skilled therapy in home health, assisted living, and community settings, serving senior and geriatric patients.

You will conduct in-home evaluations, create rehab plans, and work with an interdisciplinary team to improve mobility and independence, with emphasis on fall prevention and patient education.

Qualifications

• Licensed PT in California (Required).
• CPR certification required (AHA).
• Minimum 1 year of clinical home health experience preferred.
• Experience with senior care, geriatrics, rehab, or community-based care preferred.

Responsibilities

• Conduct in-home physical therapy evaluations and reassessments.
