Job summary

Remedy Home Health Care in East LA County is seeking a licensed Physical Therapist to join our home health team in Montebello, Downey, and Bellflower. This Per Diem / PRN role offers flexibility, autonomy, and the chance to provide high‑quality rehab and community‑based care directly to patients in their homes.

As a Home Health PT, you’ll work with senior and geriatric patients recovering from illness, injury, or surgery, delivering skilled therapy in home health, assisted living, and community

Qualifications

• Licensed Physical Therapist (PT) in California required.
• CPR certification (AHA approved).
• Minimum 1 year of clinical experience; home health preferred.
• Experience with geriatrics, senior care, rehab, or community-based care preferred.
• Strong clinical, communication, and patient education skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct in-home PT evaluations and reassessments.
• Develop and implement personalized rehab care plans.
• Improve strength, balance, mobility, and functional outcomes.
• Provide therapy in home health, assisted living, and community care settings.
• Educate patients and families on fall prevention and safe mobility.
• Document visits using EMR (WellSky/Kinnser).
• Collaborate with interdisciplinary home health teams.

Your Role: Home Health Physical Therapist (PT)

As a Physical Therapist in Home Health, you will provide skilled rehab services tailored to each patient’s needs. Your focus will include fall prevention, strength building, mobility training, and functional independence for senior and geriatric patients.
