Job summary

Remedy Home Health Care is seeking a licensed Physical Therapist (PT) for a Per Diem / PRN role in North Orange County. You will provide skilled rehab in home health, focused on geriatrics and improving mobility, with flexible scheduling and independence in community settings.

Requirements include CA PT license, CPR (AHA) certification, and at least 1 year of relevant clinical experience. A valid driver's license and reliable transportation are also expected.

Qualifications

• Active California PT license.
• Current CPR certification (AHA) required.
• At least 1 year of clinical experience; home health preferred.
• Experience with seniors/geriatrics or rehab in community settings preferred.
• Strong clinical, communication, and patient education abilities.
• Valid driver’s license and reliable transportation with auto insurance.

Responsibilities

• Conduct in-home physical therapy evaluations and reassessments.

As a Home Health Physical Therapist, you will work with senior care populations, including geriatric patients recovering from illness, injury, or surgery. Your Role: Home Health Physical Therapist (PT)

As a Physical Therapist in Home Health, you will provide skilled rehab services tailored to each patient’s needs. Your focus will include fall prevention, strength building, mobility training, and functional independence for senior and geriatric patients.

Key Responsibilities:
