Job summary

Remedy Home Health Care in North Orange County is seeking a licensed Physical Therapist (PT) to join our Home Health team. Deliver skilled rehab in home health, senior living, and community care settings to help geriatric patients regain independence.

This Per Diem / PRN role offers flexibility and autonomy, with collaboration from nurses, case managers, and physicians. You’ll support mobile home health therapy across the community and use WellSky EMR.

Qualifications

• Licensed Physical Therapist (PT) – California (Required)
• CPR Certification (AHA approved)
• Minimum 1 year of clinical experience
• Experience in senior care, geriatrics, rehab, or community-based care preferred
• Strong clinical, communication, and patient education skills
• Valid driver’s license, reliable transportation, and auto insurance

Responsibilities

• Conduct in-home physical therapy evaluations and reassessments
• Develop and implement personalized rehabilitation care plans
• Improve strength, balance, mobility, and functional outcomes
• Provide therapy in home health, assisted living, and community care settings
• Educate patients and families on fall prevention and safe mobility techniques
