Job summary

Remedy Home Health Care in North Orange County is seeking a licensed Physical Therapist (PT) to join our Home Health team on a Per Diem basis. You will provide skilled therapy in homes, senior living communities, and community settings, delivering one-on-one care to geriatric patients.

This flexible role offers autonomy, collaboration with nurses and physicians, fall prevention and mobility training, and the chance to help patients regain independence.

Qualifications

• Licensed Physical Therapist (PT) in California is required.
• CPR Certification (AHA approved) is required.
• Minimum 1 year of clinical experience; Home Health preferred.
• Experience with senior care, geriatrics, rehab, or community-based care is preferred.
• Strong clinical, communication, and patient education skills are essential.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Conduct in-home physical therapy evaluations and reassessments.
• Develop and implement personalized rehabilitation care plans.
• Improve strength, balance, mobility, and functional outcomes.
• Provide therapy in home health, assisted living, and community care settings.

Your Role: Home Health Physical Therapist (PT)
