Job summary

Remedy Home Health Care is seeking a licensed Physical Therapist (PT) for our East LA County home health team. This per diem role focuses on delivering skilled therapy to senior patients in homes and community settings, emphasizing fall prevention and functional independence.

You will work with a multidisciplinary team to tailor rehabilitation plans, improve mobility, and support geriatric patients recovering from illness or surgery in East LA County.

Qualifications

• Licensed Physical Therapist (PT) – California required.
• CPR Certification (AHA approved).
• Minimum 1 year of clinical experience; home health preferred.
• Experience with senior care, geriatrics, rehab, or community-based care preferred.
• Strong clinical, communication, and patient education skills.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Conduct in-home physical therapy evaluations and reassessments.
• Develop and implement personalized rehabilitation care plans.
• Improve strength, balance, mobility, and functional outcomes.
• Provide therapy in home health, assisted living, and community care settings.
• Educate patients and families on fall prevention and safe mobility techniques.
• Document visits using EMR (WellSky/Kinnser).
• Collaborate with interdisciplinary home health teams.

This Per Diem Physical Therapist role offers flexibility, autonomy, and the opportunity to provide high-quality rehab and community-based care directly to patients in their homes.

As a Home Health Physical Therapist, you will work with senior care populations, including geriatric patients recovering from illness, injury, or surgery. You’ll deliver skilled therapy services in home-based, assisted living, and community care settings, helping patients regain independence and improve their quality of life.

Your Role: Home Health Physical Therapist (PT)
