Job summary

Live Well Homecare in Connecticut seeks a licensed Physical Therapist to provide in-home and senior living care under our Geriatric House Calls model. You will evaluate, design and deliver evidence-based treatment plans, support mobility and independence, and document in EMR.

Qualifications

• Valid Physical Therapist license in the state of CT.
• Degree from an accredited Physical Therapy program.
• Computer proficiency.
• CPR certification required.
