Job summary

Affinity Home Health Care Inc in New Jersey is seeking a licensed Physical Therapist to provide personalized, one-on-one care to older adults in their homes or senior living communities, with a flexible schedule and telehealth when appropriate. You will conduct evaluations, develop evidence-based treatment plans, deliver sessions to improve mobility and independence, and document progress in the EMR.

Qualifications

• Valid Physical Therapy license and degree from an accredited program.
• Proficiency in basic computer use and EMR documentation.
• Current CPR certification.
• Physical ability to perform patient transfers, repositioning, and other therapeutic activities, including lifting and walking for extended periods.

Responsibilities

• Conduct evaluations and develop individualized treatment plans based on evidence-based practices.
• Deliver therapy sessions aimed at improving mobility, independence, and overall wellness.
• Document patient progress accurately within the EMR system, adhering to regulatory standards.
• Collaborate with a multidisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Provide virtual care to enhance access and continuity of treatment.
