Job summary

LiveWell Homecare Agency in New Jersey is seeking licensed Physical Therapists to deliver personalized, one-on-one care to older adults in private homes and senior living communities, using the Geriatric House Calls™ model. Qualifications

• Valid Physical Therapy licensure or eligibility.
• Degree from an accredited PT program.
• Proficiency in basic computer and EMR systems.

Responsibilities

• Provide tailored physical therapy sessions in patients’ homes or communities.
• Perform evaluations, develop individualized treatment plans, and administer evidence-based therapies.
• Support patient mobility, independence, and wellness.
• Document care accurately in electronic medical records.
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Utilize telehealth when appropriate to extend care.

Role Overview
