Job summary

LiveWell Homecare Agency in Pennsylvania is seeking a licensed Physical Therapist to deliver in-home and senior living care to older adults in the community. You will evaluate patients, develop treatment plans, provide evidence-based therapy, document in EMR, and collaborate with interdisciplinary teams and caregivers.

Qualifications

• Valid PT license.
• Degree from an accredited program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Ability to meet physical demands including lifting, transferring, and driving.
