Job summary

LiveWell Homecare Agency is seeking a Physical Therapist to provide one-on-one therapy in private homes and senior living settings. You will evaluate patients, craft individualized plans, and deliver evidence-based treatments to improve mobility and independence.

The role includes documenting care in EMR, collaborating with an interdisciplinary team, and educating patients and caregivers on safety and aging in place. Telehealth may be utilized to increase access and flexibility.

Qualifications

• Valid PT license or eligibility in the practicing state.
• Degree from an accredited PT program.
• Proficiency in computer use and EMR systems.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in private homes and senior living communities.

Role:

Physical Therapist

Responsibilities:

• Provide one-on-one physical therapy to older adults in private homes and senior living communities
• Conduct evaluations, develop personalized care plans, and deliver evidence-based treatments
• Promote patient wellness, mobility, and independence
• Document care within electronic medical records in compliance with regulations
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and fitness specialists

Physical & Other Requirements:

• Ability to perform moderate physical effort, including lifting and repositioning patients
• Capable of driving to patient locations
• Willingness to wear PPE when necessary
