Qualifications

• Licensed PT or license-eligible in practice state.

• Proficiency with EMRs and computers.
• Current CPR certification.

Join as a licensed Physical Therapist dedicated to delivering personalized, one-on-one care to older adults in their homes, senior living communities, or through virtual platforms.

Key Responsibilities

• Provide individualized physical therapy to enhance mobility, independence, and quality of life.

• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Utilize telehealth and virtual care to extend reach and flexibility.

Qualifications And Skills

• Valid Physical Therapy license or eligibility in the practice state.
• Degree from an accredited Physical Therapy program.
• Proficiency in computer use and electronic medical records.
• Current CPR certification.

Physical requirements:

Ability to perform moderate physical effort, assist with patient transfers, stand and walk for extended periods, and drive to patient locations. Must be able to wear PPE as needed.
