Chestnut Health is seeking a Physician Assistant to join our physiatry team providing rehabilitation in Skilled Nursing Facilities. You will work in a collaborative, patient-centered environment focused on improving function and independence, with a culture that balances compassion and innovation.

Qualifikationen

• Active licensure to practice in the state of employment (Illinois).
• Current DEA license required at start date.
• Current controlled substance license required at start date.
• NCCPA certification is required.
• Certified Physician Assistant credential is required.
• Desire to work in Physiatry/PM&R; experience a plus in SNF, orthopedics, or PM&R.

Aufgaben

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive rehabilitation treatment plans.
• Conduct patient assessments including histories, exams, and evaluations to set rehabilitation goals.
• Monitor progress and adjust plans to optimize functional outcomes and independence.
• Participate in multidisciplinary team meetings and educational activities to enhance clinical knowledge.
• Document evaluations, treatment plans, progress notes, and discharge summaries in EHRs.
• Educate patients and families about rehabilitation process, outcomes, and post-discharge care.
