Job summary

Chestnut Health is seeking a highly motivated Physician Assistant to join our physiatry team in a patient-centered SNF setting.

You will collaborate with therapists, nurses, and physicians to plan and implement comprehensive rehabilitation programs, conduct thorough assessments, and monitor progress to maximize functional outcomes. New grads are encouraged to apply, with a supportive, work-life balanced culture.

Qualifications

• Active license to practice in the state of employment.
• Current Controlled Substance License required on start date.
• Current DEA license required on start date.
• NCCPA Certification.
• Desire to work in Physiatry/Physical Medicine and Rehabilitation.
• Orthopedic experience is a plus.
• Physiatry experience is a plus.
• Skilled Nursing Facility experience is a plus.

Responsibilities

• Collaborate with therapists, nurses, and other professionals to develop rehab plans.
• Conduct patient assessments including histories and exams to set rehab goals.
• Monitor progress and adjust plans to improve function and independence.
• Participate in multidisciplinary meetings and ongoing education.
• Maintain accurate EMR documentation from evaluation to discharge.
• Educate patients and families about rehabilitation processes and post-discharge care.
