Job summary

Sentara Health in Chesapeake, VA is seeking a Pharmacy Technician Home Infusion Care Coordinator to provide clinical pharmacy care delivery under supervision. The role is full-time, Monday through Friday, Day Shift (8AM-5:30PM) at the Chesapeake Home Care Services Office, with base duties including preparing and delivering medications, triaging questions, and ensuring supply continuity.

A Virginia Pharmacy Tech License and a CPhT license are required, along with 2 years of pharmacy experience.

Qualifications

• High School Diploma or equivalent required.
• Virginia Pharmacy Tech License required.
• CPhT license (NHA or PTCB) required.
• 2 years of pharmacy experience required.
• Strong customer service skills preferred.

Responsibilities

• Demonstrates advanced technical skills and ownership of the work area.
• Demonstrates the ability to work independently defined areas of pharmacy practice competently.
• Assumes local and/or system responsibility for assigned tasks related to advanced clinical job positions.
• Participates in the training of new employees.
• Fills and delivers medications under the supervision of a licensed pharmacist/licensed independent practitioner.
• Ensures adequate supplies are available for continuous patient care.
• Triages and escalates any questions or issues outside the pharmacy technician's scope of practice as needed.
• Uses appropriate systems for processing patient information.
• Ensures quality assurance is maintained according to departmental and legal requirements.
• Operates pharmacy equipment appropriately.
• Participates in clinical initiatives to continuously improve patient care.
• Training in an advanced Clinical Pharmacy Technician job function.
• In addition to the above job responsibilities, other duties may be assigned.
• A pharmacy technician license is required in the state of practice.
