Advocate Health Care - Oshkosh location is seeking a Pharmacy Technician in Transitions of Care Services. The role focuses on obtaining medication histories from patients at Aurora Medical Center Oshkosh and supports the care team with accurate documentation.

Shifts include AM and PM with every other weekend, full-time, 40 hours per week. Requires active state licensure and CPhT/ExCPT certification; high school diploma or GED is required, with an associates degree preferred for technician

Formación

• Registration/Licensure with state board of pharmacy per state of employment requirements.
• CPhT or ExCPT certification.
• Associates Degree in Pharmacy Technology preferred for NC technician responsibilities.

Responsabilidades

• Completes area-specific work as outlined by standard processes and policies.
• Serves as an expert with equipment and technology in the pharmacy tech role.
• Ensures documents and records are accurate and completed in a timely fashion.
• Supports communication with patients and care team.
• Identifies quality improvement opportunities to ensure safe, effective patient care.
• Participates in site or system initiatives and education efforts.
