Job summary

Lumicera is hiring a Pharmacist, Clinical Consultation to perform telephonic patient consultations regarding specialty medications and disease state management. You will reconcile medications, screen for interactions, and provide side effect management recommendations.

The role requires clinical expertise, with a focus on accuracy, documentation, and collaboration with prescribers. A license and ACPE-accredited education are required, with a preference for experience in specialty pharmacy and

Qualifications

• Graduate of ACPE accredited School of Pharmacy with a PharmD or B.S. Pharmacy
• Current, unrestricted pharmacist license in state of residence
• At least two years of experience as a licensed Pharmacist (preferred)
• Strong clinical background required
• Experience with Specialty Pharmacy medications desirable
• Participate in and support compliance objectives
• Ability to interact professionally with colleagues

Responsibilities

• Provide telephonic patient education on specialty medications and disease state management
• Document consultations and interventions in pharmacy software per policies and URAC standards
• Interpret prescriptions and verify drug strength and directions with providers or patients
• Contact physicians for new prescriptions and for prescription clarifications
• Coordinate prescription transfers and promote generic/biosimilar options where possible
• Adapt to new workflows and stay current with specialty therapies and guidelines
• Back up pharmacists as needed and assist in training junior staff
• Report medication errors and support quality improvement initiatives
• Comply with all applicable federal and state laws and ethical standards
• Perform other duties as assigned
