Lumicera is seeking a Pharmacist, Clinical Consultation to join our team in the United States. The role centers on telephonic patient consultations for specialty medications, medication reconciliation, and interaction screening, with independent work and adherence to patient-centered care.

The position supports quality standards, aims to meet production metrics, and may involve training and back-up support for other pharmacists.

Qualifications

• Graduate of an ACPE Accredited School of Pharmacy with a PharmD or BS Pharmacy.
• Current, unrestricted pharmacist license in state of residence.
• At least two years of experience as a practicing licensed Pharmacist preferred.
• Strong clinical background; experience in Specialty Pharmacy desirable.

Responsabilités

• Provide telephonic patient education on specialty medications and disease state management.
• Document consultations in pharmacy software per policies and URAC.
• Interpret and evaluate prescriptions; verify details with providers to expedite processing.
• Collaborate with physicians to convert prescriptions to generics, biosimilars, or preferred drugs.
• Verify information entered in the system and respond to clinical questions from the team.
• Adapt to new workflows and stay current with specialty therapies and guidelines.
• Back up other pharmacists and train junior staff.
• Document and report medication errors per policy and participate in quality initiatives.
• Comply with applicable federal/state laws and uphold high ethical standards.
• Other duties as assigned.
